  • Your latest CD4 count
  • The approximate date viral load became undetectable

For people living with Diabetes

  • If you're diabetes is Type 1 or Type 2
  • Your latest HbA1c reading
  • Your latest blood pressure reading
  • Your height & weight
  • Names of the medication you may be using
  • The approximate date you were diagnosed
  • Have you ever needed time off work because of your symptoms?

For people with a history of mental health related issues

  • The approximate date you first noticed symptoms
  • Are your still on treatment? or
  • The approximate date you became symptom and medication free
  • Have you ever needed time off work because of your symptoms?

For people with a history of recreational drug use

  • The approximate date of your last recreational drug use and
  • Have you even needed time off work because of drug use?
